Cabot Public Schools is proud to announce a new partnership with Unity Health to provide virtual primary health services to all students, district staff and their families.
The clinic is located in the Cabot Health Services building at 811 S. Pine Street and is open Monday through Friday from 8 a.m. to 4 p.m.
Through the service, patients can connect with Unity Health providers via a secure, HIPAA-compliant video platform, allowing them to speak with a medical professional in real time during a telehealth appointment. Prescriptions can also be sent directly to the pharmacy of their choice.
Prior to your first telehealth session, you will need to complete a New Patient Packet. Parents, school district faculty and staff can access the form online here or in the Parent Resources, Staff Resources, and Health Services Department sections of the school district website.
For patients who already have primary care insurance, Unity Health will work with your provider to process a one-time change of coverage at the time of the visit. This service is for acute minor illnesses and injuries and does not replace your existing primary care provider.
Insured patients pay their normal copay and Unity Health bills their insurance company. Uninsured patients are supported by Unity Health and our district social workers.
