The Louisiana Department of Health is seeking provider and stakeholder engagement, feedback, and partnerships regarding Centers for Medicare and Medicaid Services (CMS) funding opportunities focused on improving maternal health.
Through the Transforming Maternal Health (TMaH) model, CMS will select up to 15 state Medicaid agencies to receive funding specifically to improve maternal health for people enrolled in Medicaid and the Children's Health Insurance Program (CHIP).The model will support selected state agencies in developing a whole-person approach to pregnancy, birth, and postpartum care that addresses physical, mental health, and social needs.
LDH Proposal
LDH will apply to CMS to be one of the state Medicaid agencies participating in the TMaH model. If selected, the agency could receive up to $17 million over 10 years. The application deadline is September 20, 2024. The awardee is expected to be announced in mid-December 2024.

Stakeholder Partner Meeting
LDH will host a series of meetings to provide information about the TMaH model and gather feedback from potential partners.

During the webinar, LDH will provide an overview of the TMaH model requirements and an outline of the NOFO proposal, and participants will have the opportunity to share feedback and suggest additional activities they believe are essential to the proposal.
